Important Reminder from School Nurse

Students are not allowed to bring medication of any kind (over-the-counter or prescription) to school without the proper paperwork submitted to the school nurse.  If you have any questions please feel free to call Elementary office (968-3014) or Secondary Office (968-3902) 

 It is a state-mandated law that students who need any medication at school, whether it be prescription or over-the-counter medicines need to have the appropriate medication form completed by the student's doctor and signed by a parent or guardian each and every year. This includes inhalers, epi-pens that are kept by the students, Tylenol, and even cough drops, etc.

The Administration of Medication forms can be picked up in the school offices or can be viewed and printed from the link shown below. You may also have the doctor fax his/her information directly to the school at the following fax numbers:
Elementary School FAX # 968-3630
Secondary School FAX # 968-3370.

If your student has a potentially life-threatening condition, such as asthma, diabetes, insect or food allergy, or heart condition, a care plan form must also be filled out.
